According to a report in JAMA journal, three quarters of the patients who died of new coronavirus infection had at least one comorbidity, and hypertension was the most common. In addition, there was a significant delay in ventilator use in these patients, with only about one in five patients using the ventilator before death.
Hypertension accounted for 50.0% of deaths, followed by diabetes (25.0%) and ischemic heart disease (18.5%), the researchers reported.
There is a small proportion of ischemic stroke and congestive heart failure.
Previous cases have shown that severe patients in ICU have more hypertension than those who are not in ICU.
Previous studies have found that hypertension is not usually an independent risk factor for mortality in septic patients.
The study included the clinical characteristics of 168 new coronavirus pneumonia patients who died in 21 hospitals in Wuhan from from January 21 to 30, 2020. Three quarters of the patients were male, with a median age of 70 years; 95.8% of the patients were over 50 years old.
In addition to assessing the burden of comorbidity, the researchers also found that although all patients received oxygen therapy, 27.4% of patients only needed nose or mask oxygen. One third (34.5%) received high flow nasal oxygen therapy, 42.9% received noninvasive ventilation, only 20.2% received invasive mechanical ventilation, and 2 (1.2%) received extracorporeal membrane oxygenation.
This shows that many patients have delayed intubation, the researchers analyzed, and the potential reasons for the delay are the lack of ventilator and the lack of clinical training with doctors in the application of respiratory support devices.
